On my original iPad I have the Targus Truss case, which has padding plus
a hard leather outside. I have had the iPad slip out of my arms while I
was standing twice in the 15 months I've had it, once on a hard bathroom
floor, once on a hardwood floor, with no damage either time. I don't
know if they have updated versions for the newer iPads, but you could
always slip a sheet of foam behind the iPad to make one of the thinner
newer iPads fit securely.
Remember, though, that adding padding plus rigidity will increase the
weight, so be prepared for that.
