Notes are saved based on what you have selected in Settings/Notes/Default
Account. Also, what you have turned on in Settings/Mail, Contacts, Calendars
determines what syncing is happening and combined with the Notes default
account setting determines where your notes are saved. (Of course if you're
syncing to Outlook through iTunes on a computer, that can create another set
of issues.)

I haven't had any problem, going back to 2009, several versions of iOS,
several devices, and two versions of Windows & Outlook.
