MEID is linked by the service provider to the SIM number and telephone number. This is true for both post paid numbers ( for billing to an address ) or pre paid number ( for maintaining balance account).
That is the reason  the stolen or lost handsets can be traced by the service provider if they are again used on the network.

On Verizon (I'm pretty certain) and sprint (I know for sure) the phone must be white listed first. Can't just toss the sim in.  They have to load the MEID into the system.

I expect to be replacing an existing phone on AT&T shortly.

I expect to go to my local Apple Store, buy the iPhone using Apple's iPhone Upgrade Program, pay monthly payments to Apple, and simply swap the nano-SIM out of the old iPhone and into the new one, and then follow the on-screen directions on the iPhone to activate it.

No visit to an AT&T Store at all.
