I used to have it but can't find it now. What the name exactly so I can do a search on iPad Air running the latest software. I probably tucked it into a folder somewhere. I checked setting but don't see anything called Health.

Try doing a search on the iPhone for Health, it may show up under applications.  Worth a try before you reinstall the latest iOS.
