Interesting, and discouraging.  I guess that means the service is designed to be always mediocre.

Assuming, hypothetically, that cost is no object, what's the best way for a residential user to get the fastest overall speed?

Fibre may or may not increase speed, yes, it depends on the site serving the web page or file. My DSL is only 15mbit and many downloads are at line speed so those may increase a little or a lot. The chief benefit of fibre is for multiple users
