I have also checked books out from the library and had them sent to my Kindle on the iPad. Once you go into Manage my Kindle through a web browser and select Loans and delete from the library. You then need to open the app on your iPad and make sure it syncs. Once the sync occurs the book should no longer appear. I just verified this since I had a couple that needed deleting.
If this doesn't fix it then I suggest you follow David's suggestion to reset the iPad.
