Dolby Volume solved this for me quite a while ago.  A "feature" in my HT system.  Something to consider if you are replacing a HT receiver.  Have also seen it in a high-end TV.

Damned annoying, though, and so I've told a station or two -- pointing out
that tuning out/muting/changing stations isn't what they want their
listeners doing, is it?

A few years ago, the FCC passed a rule that the audio on commercials could
not be any louder than the program that was playing. Recently, I read that
to report rule breakers note the channel;e.g., WRAL, the company
advertising, and the time.
The audio level has been creeping up.
