From what Jim said it seems Mac iTunes does not handle nested folders either. So, I can only assume that iTunes photo sync was designed solely for users who have few photos

iTunes is an Apple software whereas Windows is Microsoft product.
The work you want iTunes to do, was not part of original desire list of iTunes developers.
iTunes has undergone 10 revisions but still falls short of some desires by its users.
The reason is the fact that the priorities are not same.

I agree. M$ software does dominate globally. Still not sure who or why Windows iTunes falls short
