I'd guess that they are using an ad placement service, as I do on a couple of my websites. The ad providers, Google AdSense in my case, should ideally screen out any junk before it goes live, but it's a mostly-automated system. It often relies on reports from site visitors to site admins, then back to the ad network, and you can imagine the time it takes to finally get something done about it.
