Check *all* of your sync settings -- every tab, every item.

Consider using playlists to keep track of groups-of-items (music, video,
movies, tv shows, podcasts, audiobook, maybe ebooks) that you have on the
iPad. I don't use "smart" playlists, I organize each one the way I want at
any particular time -- yes, it does take a bit of housekeeping but saves
checking on checkmarks, dates-played, etc. for a zillion items in my
library. (I don't know how playlists handle iBooks, my usual ebook reader is
Kindle & it handles things itself.)

I have never had a problem removing apps from a device but keeping it on my
computer. Not every app will work on every device I have, nor do I
necessarily want every app that will work on a device *on* a device for
whatever reason (mostly -- I have *way* too many apps!).

I have four different devices & let iTunes handle the syncing; I haven't had
any of the types of problems that you have. I just have to check sync
settings carefully if I want to make changes on any items. Sometimes I
forget one, or I forget to re-order a playlist, so I have to re-sync but
that's no big deal.
