I'm an AT*T U-Verse customer. Our home TV and WiFi is from the same router, a 2Wire brand that AT&T provided.
I would only get the extreme or express if I wanted to extend the WiFi signal out of the range of my home and yard.
If you do not want to lease your router from ATT to include wireless, then the Airport Express or extreme would be hard-wired to the Ethernet port in the Airport from the router.
Main differences in the 2 are the express is 1 Ethernet port, a USB and a jack for a speaker thing. The Extreme has 4 Ethernet ports so you could hardwire 3 other computers to it and still have surrounding Wifi, and the USB can be linked to a USB Printer or hard drive.
I've been happy enough with just the ATT router in my 2200 sq/ft house, but I do have an extreme base from a couple of years ago that I can use if I want to make a 'Bridge' and extend my signal from the AT&T router itself.
