It was neurosurgery 10 years ago that caused it.
Had little use of the arm and hand for 2-3 months, then gradual return of function as the nerves grew back down the length of the arm. It's pretty much only the intrinsic muscles of the hand that are weak now, and have been unchanging for several years.
