An iTunes gift card is a flexible choice. He will be able to buy iPad apps, music, movies or anything else at the iTunes store without having to use your user account.
You can also gift a particular app through the iTunes store if there is a particular one that you have in mind. You will need an account at iTunes to do this, however.
The grandkid will have to set up an apple ID once he receives his iPad for Christmas, he will receive the gifted apps via an email and I believe he can then redeem them on his iPad thru iTunes on his computer by typing in the redemption code for the apps you purchase as a gift.
I would check the Apple site for info on gifting apps, as it is now an option I believe.
